How would I make this and are there any patterns out there to use?

Find a copy of the Confederate Sketchbook and you'll do well. It's very cheap, and I think I paid $3 for mine when I bought it a few years ago.

The main problem with making a cap pouch is that you'll need a form or mould to shape the pouch for most varieties. I made something awhile back that's basically a piece of wood the size and shape of the pocket that's screwed to a flat piece of plywood. Another piece of plywood has a cutout that matches the pocket-shaped piece of wood, but a little larger to allow for the thickness of the leather. I then wet a suitably thin piece of leather and clamp it between the wood until it dries to get the correct shape.

Oh, and it's a bit complicated if you don't have much leatherworking experience.
