I bought a cheap, small bag. It has a sheath made into the strap for a patch knife, but no loop for a short starter.
My starter is the type with a round wooden ball for a handle.
Should I try to put a loop on the bag, a lanyard on the starter, or just leave it where it is now, in the knife sheath?

I sewed a sleeve inside my bag. It's a little less convenient perhaps, but the starter is always in the same place and doesn't fall out.

First thing you may want to do is square up that round ball by cutting the top and one side flat. It is a lot easier on your hand when trying to just start the ball below the muzzle hitting your hand on a flat surface than on that round ball. This comes from experience. As far as a loop, i take a piece of scrap leather sand stitch it to the side of the bag, that is to the front when i am wearing it, and place the dowel of the starter on the end of the bag and fold the leather over it to see how wide to make it and stitch the other side. I make it 4"=6" long. Hope this makes some sense to you.
