Placed a patch on my rod to swab the barrel. When the rod was removed, there was no patch. I tried to snag it with my coiled type of patch remover. Cannot get it out. I'm sure this has happened to someone here before, so, give me some idea of the next thing to try. The patch was lubed with Moose Snot prior to inserting if that makes any difference. I also tried a bore brush to no avail. Ideas anyone?

Use a smaller brush, like a 22 cal. Hit bottom then twist a few times, then gently pull it out. Took me a couple times last week when i had a 12 bore patch get away from me.
For a second I thought you have been shooting using the same patch over and over again and now finally lost it lol.

I had this problem a lot of times in sixty years but always solved by using a bullet puller made with a fine screw that can goes into the powder chamber if necessary...
You can also use the air pressure of a small air extractor (home compressor ?) in the hole of the pan or the hole of the nipple wen it is not a flinter...

The patch could be stuck to the wall of the barrel and you just keep going past it.
Things to try,
Run another unlubed tight patch down the barrel to remove it from the side wall if that's where it is. Then try the worm again.
If that doesn't work you may have to re-clean with hot soapy water.
Sometimes you can shoot a dry patch out by putting some powder under the nipple. but then you have to re-clean again.

Bullet pullers do a good job of pulling patches and that's what I normally use. A small dia. brass brush does a really good job, too.

I've found the multiple bristles on an underbore brass brush will grab easier than a bullet puller. had this happen last week, got it the 2nd try.

But that's part of the sport! I learned from that, but still lose one every once in a while.
Most of the time it's caused by pulling out of the bore too quickly, I see the patch and as the jag clears, the vacuum sucks the patch back in. Usually happens when I forget to pull the hammer back to half cock off a nipple with a leather under it when wiping before a range trip.

I have found the dual wire patch pullers to be ineffective. I have taken a coat hanger or other stout wire and simply bent a loose corkscrew in the end. Has worked every time.
After a lot of trying various methods, I successfully removed the patch. Tried a undersized bronze bore brush, bristle brush and patch puller. Nope. Tried cleaning the bore. Nope. Finally after the final cleaning patch was pulled out the little wad of patch fell out. Thanks for the suggestions
