I concur with replacing the factory nipple. Buy a new "hot shot" type of replacement nipple. Plus, when you buy a known good quality nipple, at least you've eliminated one source of the problem.
 
Sometimes it's the lube mixing in with the S/K2NO4/C combustion making a bunch of partially burned gunk.
Some things that might help:
A dry patch over the powder before seating the lube patched ball.
Or less lube.
Or add some dry lube.
Or a water based soap lube instead of grease/waxy lube.
