I say this belt pouch and would really like to make one. It looks very straight forward, however, how does it stay on the belt? Does it just tuck in or is there a type of belt loop to hold it?

Likely has a strap to accommodate a wide belt sewn to the back.

In construction, after the parts of the bag are laid out, the strap (and other adornments) is sewn onto what will be the outside of the back (and the adornments on the outside of the face) prior to sewing the bag together, inside-out

I've seen a lot more of those type of bags with no loops on the back. I think a lot more of them were just worn wrapped around the belt and used for lighter stuff, like tobacco, edible stuff, wasp nest for wadding, dry stuff for fire starting and a lot of other possibilities.
 
I've seen long (12"+) unadorned ones that the guys just put the toteables in then pull it up through their belt and fold the top down over the front. They seem to stay put.
