need help id'ing a caplock double shotgun

that's as far as i can get without having my sweaty hands on it. as clean as the barrels are and the rest i am slightly suspicious of it. would sure like to knock over a big Tom with it though. that inlaid cartouche between the Barrels has to stand for something but i sure haven't found it yet. have been to the little sign that says "you have reached the end of the information highway, now turn and go back" and still haven't nailed this gun down.
