I think Mike has it right. (@45D ) your bolt isn’t resetting. Your caps aren’t fitting the cones properly which means they are holding the hammer back by a couple thousandths of an inch. Locks up a revolver every time.

I just remembered I have a Colt Signature series that will ONLY take (due to clearance Remington # 11's )
As you see they are SHORTEST in height of all the caps measured at the time this chart was produced
I just bought 1000 Remington #11's that they said they just got them in for $8.99 and fair shipping price
I have never bought from these people before but I checked them out and they seem legit
