If the gun has been shot for over a decade one would think the sharp edge of the rifling or muzzle would not be an issue…

Is it possible the patch material is old and just being slightly cut while loading…🤷‍♂️

Try using additional lube on the patch that may help as well.
 
I would first wrap some 4/0 steel wool around a brush and give it 100 or so strokes up and down. That was the recommendation of the owner of GM back in the early 90's. That barrel is now very accurate and no cutting anymore.
